Apply to become a puppy carer

Thank you for your interest in raising a Seeing Eye Dogs puppy! We can’t wait to welcome and support you on this incredible and rewarding journey.

To submit your application, please complete this form and indicate whether you are interested in caring for a period of 6 or 12 months. This form is to be completed by the primary carer only — the person in your family who will be responsible for the puppy for most of the time.

Once we receive your application, a member of our team will notify you if you’re in an area that needs Puppy Carers. We will also help you to complete the following: 

• External Risk Assessment Form 
• Global Carer Agreement 
• Working with Children check (WWCC) in VIC and NSW / Blue Card in QLD 
• Police check 
• Interview and Property Inspection
